Please see attached templates for your annual pesticide report. There are 3 attachments:

• Pesticide Report Template - Excel - we encourage those that are able to complete the report digitally using the Excel report template and emailing your completed report to tgalvin@falmouthme.org
• Pesticide Report Template - PDF - for those who are more comfortable handwriting your report, please print and use this PDF template and mail your completed report to 271 Falmouth Rd. Falmouth, ME 04105
• EXAMPLE Completed Pesticide Report Template - this provides an example of how to enter your information into your report, please do not use this form to complete your report
